TERMS AND CONDITIONS

THE EMPLOYEE WILL: Support Officer
1.enter into the services of the employer as a/an Systems in accordance with the Training Plan attached hereto as Appendix A;

2.faithfully perform his/her duties and follow the regulations of the program and the placement;
3.arrive at the placement at die times specified and be in attendance for the hours agreed upon each day;
4.call the employer if he/she is late for work or unable to work for any reason (eg. sick):
5.not be paid for any overtime since the program does not provide for this;
6.contact the delivery organization regarding any problems or concerns that may arise during the placements;
7.inform the placement supervisor immediately if involved in an accident or injury during working hours;
8.at any time be able to terminate this Agreement by giving written notice stating the effective date and reason: however, re-entry to the program may be affected.
 
THE PROGRAM DELIVERY ORGANIZATION WILL:
1.provide such necessary services as counselling, work experience placement with on-the job training for __5 weeks, placement monitoring (which may
include on-site visits) and post-program follow-up to the employee;
2.ensure
that Appendix A is complied with; die Employer Application;
3.complete and send in all Workers Compensation claims to the appropriate source within 48 hours of any accident or injury involving the employee while on the
work placement;
4.be able to terminate this Agreement by giving written notice stating the effective date and mason;
5.inform the employer of the reason(s)
for the termination, if the employee terminates his/her employment;
6.inform the employer if off-site visits to the delivery organization by the employee will be required in order to provide extra support to the employee during the
placement;
       
THE EMPLOYER WILL:
1.employ the employee to work as a/an Systems Support Officer
  (job title)
2.provide the employee with effective and direct employment experience while under his/her supervision, as outlined in the Employer Application;
3.ensure that the employee receives adequate day-to-day supervision, regular continuing instruction,
and is given sufficient opportunity to learn the duties of the
job as outlined in the Employer Application;
4.provide appropriate advance instruction and ongoing supervision to ensure safe handling and use when the employee is assigned to work which involves die use
of tools, machinery or requires specific skills training;
5.provide the employee with all the necessary equipment to perform tasks safely, particularly those outlined in point four above, prior to performing the tasks;
6.be responsible for keeping an accurate attendance record of the employee, and for informing the appropriate program delivery organization of the employee's
hours as requested.
7.inform the delivery organization immediately, or in advance of the reason(s)
for dismissal, if the employee is to be dismissed;
8.inform die delivery organization immediately of any accident or injury to the employee while on die job in order to comply with Section 121 of the Workers'
Compensation Act;
9.maintain adequate third party liability insurance coverage for the employee;
10. ensure that the employee's wages am not supplemented by the employer while on the program;
11. ensure that the employee does not work more than die agreed upon weekly hours, as stated in Appendix A, since the program does not provide funds for overtime;
12. provide ongoing evaluation information to the delivery organization regarding the employee's performance;
13. inform the delivery organization regarding any changes to the training or conditions agreed upon;
14. ensure that no regular full-time employees are displaced in any way by the introduction of the employee on this work placement:
15. comply with the Occupational Health and Safety Act.

Failure to comply with the above stated terms and conditions may result in immediate termination of the placement by the delivery
Organization and elimination from future use of the program.

This Agreement will be effective from May 29,/ 95 to June 30 / 95
Notice: Personal information is maintained confidential and is used for the administration and operation of the
Training For Immigrant Professionals program.

I HAVE READ, UNDERSTAND AND ACCEPT THE TERMS OF THIS AGREEMENT AS STATED ABOVE.
