Applying for a Republic of Guyana Passport
Application forms for a new Guyana passport can be obtained from the Consulate General of Guyana, Toronto or the Guyana High Commission in Ottawa.
Mailing requests for forms must be accompanied by a self addressed envelope with appropriate postage stamp.
Please read the instructions on the back of the form carefully. Complete the form and sign in the spaces indicated at the bottom of page 1 as well as section B on page 3.
The main points to be noted are:
Lost Passports
Application for a replacement for a LOST PASSPORT must be accompanied by a sworn notarised statement explaining how the passport was lost. It must also be reported to the police.
According to the Passport Regulations, a period of nine (9) months must be elapse before a lost is replaced.
The completed application form should be submitted with four (4) photos which must be signed by a guarantor(photos+form). Other documents to be submitted are birth certificate, identification card, landed document or citizenship card.
Application by minors
Applications from persons under 18 years of age will not be entertained unless they have the written consent of the father or the other legal guardian. The birth certificate (if born in Guyana) or Statement of Live Birth (if born in Canada) and father's passport should be submitted, or otherwise a legal document to prove legal guardianship along with passport of legal guardian.
This requirement also applies where the child is to be included on the passport of his/her mother, brother, sister etc. There is a fee of Canadian $5.00 for the inclusion of each child name. Three (3) photographs of each child are also required to process the child's endorsement in the passport.
Change of name
In the case of a woman whose name has been changed by marriage, the submission of the marriage certificate and her husband's birth certificate or his passport are necessary.
NOTE: Originals of all certificates and legal documents must be submitted.
The application form for the renewal of a Guyana passport may be obtained at the Guyana Consulate in Toronto or Guyana High Commission in Ottawa.

Three (3) full face black and white or color passport size photographs taken on a clear background are required. Photos must be less than six (6) months old.
Applicant should sign in the two spaces on the form indicated for applicant's signature.
The cost of renewing a Republic of Guyana passport is $31.00 Cdn. which should be paid in cash, Money Order or Bank Draft payable to the Consulate General of Guyana. PERSONAL CHEQUES ARE NOT ACCEPTED.
The applicant will have to submit his/her present passport to the Consulate along with the completed application form and fee.
The processing time for a passport renewal is seven (7) working days.
If a child's name is endorsed on the passport, three (3) photographs of the child should also be submitted with the renewal application.
Damaged Passports
The processing of applications for replacement of damaged passports takes three (3) to nine (9) months depending on the extent of the passport and the circumstances causing damage.
Emergency travel documents are issued to persons without a valid passport who have to travel urgently for emergency purposes. Such documents are not issued for persons travelling on vacation.
Two (2) recent photographs, completed application form and fee of $10.00 Cdn. are required. The travel ticket or itinerary from the travel agent should be submitted to confirm the dates of travel.
Such travel documents are normally issued when an application for a new passport is pending and the new passport cannot be issued in time for emergency travel dates.
Applications for Birth/Marriage/Death Certificates are available at the Consulate. It is recommended that the completed application form be sent to a relative or friend who can submit it to the Registrar General's Office and collect the certificate in a few days. The fee is $30.00 Guy.
Alternatively, if there is no one in Guyana to assist, the application could be sent through the Guyana Consulate. In this case, the fee is $10.00 Cdn. and is to be paid by international money order payable to the Registrar General of Guyana. This method takes longer, approximately 6-12 months.
Full Name of applicant
Date and place of birth (certified copies of both certificates to be provided).
Country of Citizenship
Passport number, place and date of issue (certified copy of passport to be provided).
Statement that applicant is not currently married and never have been married/or was divorced or is a widow/widower (specify whichever condition is applicable)
Full name, martial status (i.e. was never married, is divorced or widow/widower), citizenship of intended spouse.
The applicant must swear to an affidavit containing the above details and submit the affidavit together with certified copies of the birth certificate and passport to the nearest Guyana Consulate or mission for transmission to the Ministry of Foreign Affairs.
Please note that the affidavit (once properly drafted and typed by the applicant or his/her legal adviser) can be signed and notarized at the Consulate. The original birth certificate and passport should be submitted to the Consulate to be certified.
The Consulate notarises legal documents such as powers of attorney, affidavits for use in Guyana. The fee is Can $5.00 per copy.
Documents must be drawn up before approaching the Consulate. The document should not be signed by anyone before coming to the Consulate. It has to be signed and witnessed at the Consulate. The person signing must submit proper photo identification such as a passport or ID card.
The Consulate witnesses signatures on authorisations for banks in Guyana. This service is free of charge. However, we only witness signatures for up to a limit of GUY$50,000 on any single authorisation.
Life certificates are notarised free of charge for pensioners who are receiving their National Insurance Scheme (NIS) pensions or Government service or sugar estate pensions. The pensioner must bring their passports or other photo identification.
These pensions are not remitted overseas directly by NIS or Government but are paid in Guyana to a bank account or to an authorised person. The pensioner is responsible for transferring the funds from Guyana.
Note: Old Age Government pension is not paid to pensioners who do not live in Guyana.
Applications for police clearance from Guyana can be obtained from the Consulate. Three (3) recent photographs are required.
The applicant must also have finger prints taken at the International Fingerprinting Services, Tel: 416 293 2030 or 416 293 1218
The photos, completed form, finger prints and fee can be submitted by a relative or friend in Guyana on your behalf. By this method, the police clearance can be obtained within a week by the authorised person. The fee is Guy$500.00.
If the applicant has no one in Guyana to submit the application, it may be submitted through the Guyana Consulate. In this case a fee of Canadian $10.00 must be paid in the form of an international money order payable to the Commissioner of Police. This method however takes longer, sometimes three to six months.
